Intravascular Laser Irradiation of Blood (ILIB) on Sleep Quality Improvement: A Randomized Placebo-Controlled Clinical Trial

Introduction Sleep quality is a fundamental determinant of human health and well-being. Modified Intravascular Laser Irradiation of Blood (ILIB), a non-invasive therapeutic modality, has emerged as a potential intervention for sleep-related disturbances. Proposed mechanisms include reduced blood viscosity and platelet aggregation, activation of superoxide dismutase, increased oxygen bioavailability, enhanced microcirculation, elevated serotonin levels, and decreased cortisol concentrations—physiological processes intricately involved in sleep regulation, mood modulation, and the stress response. Objective To evaluate the effects of Modified Intravascular Laser Irradiation of Blood (ILIB) on sleep quality in individuals with self-reported sleep disturbances. Methods A randomized, placebo-controlled clinical trial was conducted with participants who reported poor sleep quality. Subjects were randomly assigned to one of two groups: the intervention group received ILIB using a 660 nm red laser, while the control group received a placebo treatment (light emission with sub-therapeutic power, <1 mW). Both groups underwent the same treatment schedule. Sleep quality was assessed at baseline and after six treatment sessions using the Pittsburgh Sleep Quality Index (PSQI) and the Epworth Sleepiness Scale (ESS). Results Participants in the ILIB group showed statistically significant improvements in the primary outcome of global sleep quality. PSQI scores decreased from 10.24 at baseline to 6.47 post-treatment. ESS scores showed a non-significant change from 10.44 to 10.12. These results suggest enhanced overall sleep quality and reduced sleep latency, although the observed reduction in daytime sleepiness did not reach statistical significance. Conclusion Modified Intravascular Laser Irradiation of Blood appears to be a promising non-invasive approach for improving sleep quality. The clinical outcomes observed are comparable to those reported in both pharmacological and behavioral sleep interventions, particularly in terms of PSQI improvements. These preliminary findings support the need for further research to clarifyunderlying mechanisms, optimize treatment parameters (e.g., dosimetry and duration), and expand outcome assessments to include biomarkers and polysomnographic data.

Efficacy and Safety of Pulsed Magnetic Therapy in Sleep related Disorders: A Remote, Randomized, Double-Blind, Placebo-Controlled Trial

Background/Aim Sleep disturbances are common and are associated with impaired daytime functioning, reduced quality of life, and increased health risks. Non-pharmacological neuromodulatory interventions have gained interest as alternatives to hypnotic medication. Pulsed electromagnetic field (PEMF) therapy has been proposed as a non-invasive approach to improve sleep quality and recovery, but evidence from large, well-controlled trials remains limited. The objective of this study was to evaluate the efficacy and safety of pulsed magnetic field therapy (PMT) on sleep quality, sleep-related symptoms, daytime functioning, and well-being in adults with heterogeneous sleep complaints.  Materials and Methods In this remote, randomized, double-blind, placebo-controlled trial, 217 adults reporting non-restorative sleep, difficulties initiating sleep, or frequent nocturnal awakenings were assigned to active PMT using a PEMF device (Night Harmony Female/Male protocol) or a sham application for three weeks. Primary outcomes were changes in the Pittsburgh Sleep Quality Index (PSQI) global score and self-rated sleep satisfaction, restfulness, and sleep initiation difficulties. Secondary outcomes included sleep diary measures, daytime functioning, well-being (WHO-5), symptom burden (MYMOP), and daytime sleepiness (Epworth Sleepiness Scale). Analyses followed the intention-to-treat principle using ANCOVA with baseline values of the respective parameter as covariate.  Results Both groups showed significant improvements over time in global sleep quality and most secondary outcomes (p < 0.001). Between-group differences in PSQI global score were not significant (p = 0.314). However, active PMT resulted in significantly greater improvements in subjective sleep satisfaction (p = 0.02) and restfulness (p = 0.02), particularly among participants with moderate to severe baseline sleep disturbances (p < 0.001 for both). Sleep continuity measures improved similarly in both groups. No serious adverse events were reported.  Conclusions PMT produced modest but significant improvements in subjective restorative aspects of sleep beyond placebo effects and was well tolerated. Further studies using objective sleep measures and longer follow-up are warranted.

The Relationship Between Abnormal Sleeping Patterns and ADHD

The objective of this focused literature review includes establishing and explaining the high correlation between sleep issues and ADHD. Through a systematic review of sources, a high correlation between sleep issues in those with ADHD was established with multiple theories lending explanation to this comorbidity. Neurological pathways in the brain already afflicted by the disorder of ADHD may cause direct effect on sleep pathways in their dysfunction. Other comorbid sleep conditions were found to occur in high frequency with ADHD however, a common etiology of these issues is yet to be established. The major conclusion of this study includes that there are multiple mechanisms through which sleep disturbances may be caused in ADHD patients, all of which must be considered in future research.

Assessing the risk of Obstructive Sleep Apnea in Type 2 Diabetes Mellitus patients in India

Background/Aim Obstructive Sleep Apnea (OSA) is a prevalent disorder characterized by recurrent respiratory disturbances during sleep. Patients with Type 2 Diabetes Mellitus (T2DM) and obesity exhibit a substantial susceptibility to OSA (23%–86%). People with OSA have a high risk of several comorbidities like insulin resistance, cardiovascular disease, depressed mood and hypertension. Thus, the objective was to comprehensively evaluate the risk of OSA among T2DM patients in India. Materials and Methods A cross-sectional survey was conducted across four cities in India involving 2,000 T2DM patients. The survey gathered data on patient demographics, clinical endpoints, and estimated the risk of OSA using an app which included the STOP BANG questionnaire. Multivariate logistic regression analysis was used to evaluate the association between OSA risk and key variables such as age, gender, BMI, and HbA1c. Result Overall, 63.9% of T2DM patients were identified as high risk and 27.3% were at intermediate risk for OSA development. Results of the multivariate logistic regression demonstrated that patients with high BMI ≥35 had significantly greater odds (OR: 5.70; p<0.00) of developing OSA; males had 2.75 times higher odds (p<0.00) and patients with HbA1c value >8% had higher odds (OR: 1.22; p<0.00) of developing OSA. Conclusion OSA risk and prevalence are significantly higher in T2DM patients than in the general population with a notable escalation in patients who are overweight/obese, older, and have prolonged diabetes duration. Early screening using digitalization with a highly sensitive, cost-efficient, and valid tool like STOP-BANG followed by appropriate intervention for OSA can not only reduce the eventual economic burden but can improve patient outcomes.

The Potential Usefulness of Peripheral Somatosensory Stimulation in Improving Sleep Quality in Patients with Insomnia

Background Insomnia and other sleep disorders represent a major cause of disability and impaired productivity. We evaluated the impact of peripheral somatosensory stimulation (PSS) on sleep quality in 12 patients with varying degrees of insomnia. Methods Twelve adult patients underwent daily PSS therapy for a 4-week period and were evaluated using the Insomnia Severity Index at baseline (prior to initiation of therapy) and then at the conclusion of the treatments. All data were obtained through a self-reported 7-question survey evaluating overall severity of insomnia symptoms and the impact of sleep patterns on satisfaction with sleep, daily functioning, and overall quality of life. Changes from baseline insomnia scores were analyzed using cumulative link mixed models (CLMMs). Results Seven men and five women completed one month of PSS therapy. Mean age was 55.1 (range 29 to 80 years). No adverse events were described by the patients. The average total change from baseline score was -10.3 points (baseline: 16.5 vs. week 4: 6.2). Patients had statistically significant improvements for every individual survey question by week 4. The median composite score was improved from baseline, with an overall median score of 2 (IQR: 1.25 – 2.75, min-max: 1-4) at baseline compared to 0.5 (IQR: 0 – 0.25, min-max: 0-2) by week 4 (MD = -1 95% CI: , p < 0.001), signaling typically moderate insomnia at baseline vs. typically minimal to no symptoms by week 4. The predicted probability of obtaining the best outcome (score=0) was 9% at baseline vs. 53% by week 4. The overall cumulative odds ratio was 11.9 (p < 0.001), suggesting that on average, the odds of moving from one score to a lower (improved) score at week 4 compared to the baseline are approximately 12 times higher than moving to a neutral or worse score. Conclusions PSS stimulation appeared to have a significantly favorable effect on sleep quality in this group of patients. Symptoms related to ability to fall asleep, remain asleep, and overall quality of sleep were all improved with PSS therapy. We suggest that further investigation into the potential usefulness of PSS therapy in patients with sleep disorders is warranted.

The Role of Non-Pharmacological Interventions for Disrupted Sleep in the Moderate-Severe Dementia Population: A Systematic Review

This study systematically reviews the literature on non-pharmacological interventions for disrupted sleep-in people meeting established criteria for moderate-severe dementia, and to analyze the methodological quality of the included studies. The PubMed, PEDro, Cochrane, Virtual Health Library, APA PsycInfo databases were searched using a systematic literature review approach to identify various types of non-pharmacological treatments that improve disrupted sleep-in subjects with moderate-severe dementia. In accordance with the inclusion criteria, eight studies were systematically reviewed and analyzed according to the type of non-pharmacological treatment carried out. This systematic review showed that 50% of the studies used bright light therapy, 12.5% ​​the use of manual therapy, and 37.5% sleep hygiene or walking or a combination of these interventions. Based on the results of the present study, although there is some evidence to support these strategies, it is not significantly supported and highlights variation in the way the interventions were delivered. Disrupted sleep is highly prevalent in people with dementia and have a negative impact on the quality of life of the sufferer and the caregiver. Non-pharmacological approaches to its treatment are increasingly popular as an alternative to drugs, whose efficacy and side effects have raised concerns among the population. Currently, there is a need to carry out more future research to establish its effectiveness and to be able to provide clear guidelines at the time of clinical practice.

Effect of Ultra-Processed Foods Consumption on Sleep Disturbances Among Brazilian Adults’ Population: A Propensity Score Matching Approach

Background Diet, exercise and sleep are three pillars of a healthy life. These lifestyle factors influence one another in complex and dynamic ways. Consistent evidence exists about the bidirectional relationship between sleep patterns and food intake. Objectives We aim to assess the effect of the ultra-processed food(UPF) consumption on sleep disturbances in a sample of the Brazilian adults. Method We analyzed 77,298 persons aged 18 and older from the Brazilian National Health Survey (2019). A Propensity Score Matching (PSM) approach was used to address confounding bias and balance the matched exposed and control groups. The population average treatment effect (PATE) and the population average treatment effect on the treated (PATT) were estimated. Also, the Absolute Risk Increase and the Relative Risk Increase were computed. Results The findings revealed that Brazilian adults who consumed UPF experienced small but statistically significant sleep disturbances compared to non-UPF users. Furthermore, salty UPF showed small but significant worsening of sleep compared to sweet UPF users. Conclusion The consumption of UPF is associated with sleep disturbances in Brazilian adults. The consumption of salty UPF had a more significant effect on sleep disorders than sweet UPF in the overall population. However, among individuals who already consumed sweet or salty UPF, these foods had a more significant effect on sleep disorders. To gain a deeper understanding of the complex relationship between UPF consumption, sleep disorders, and their impact on health and quality of life, further research is needed, including precise measurements and consideration of personality traits using prospective designs.

Sleep Disturbances and Hip Fractures

Hip fractures, which remain highly prevalent among the elderly and produce enormous social and economic costs, have not truly abated in prevalence despite years of research that outlines a multitude of preventable risk factors and intervention programs. This review aimed to examine if sleep disturbances have a bearing on the risk of incurring a hip fracture directly or indirectly, among older adults. The specific aim was to explore and summarize what we know, and how this might inform future research and practice. To this end, PUBMED, Web of Science, Scopus, and Google Scholar data bases were searched to uncover available data representing the topic of sleep in relation to hip fractures among the elderly. Articles of note were scrutinized and summarized in narrative form. Results showed very few studies on the topic prevail, even with no restriction on years examined, and of these, discordant, rather than any solid uniform conclusions prevail. It is concluded there is a need to explore this topic carefully, including both the direct, as well as the indirect impact of sleep that may place an older adult at heightened risk for a fall and hip fracture injury as well as subsequent fractures.

The Intersection of Cultural Characteristics and Genetics on the Prevalence of Delayed Sleep Phase Syndrome in Brazilian and Japanese Adults

Delayed sleep phase syndrome (DSPS) is a circadian rhythm disorder where individuals experience difficulty modifying the time they go to sleep and wake up in response to environmental changes. The circadian rhythm itself is regulated by a variety of clock genes, and various other genes (e.g., AA-NAT gene, CKIϵ gene) code for proteins that regulate clock genes. Various polymorphisms of the clock gene influencers have been shown to increase susceptibility to DSPS. This paper seeks to examine how certain cultural characteristics (e.g., napping, timing of meals, exposure to artificial light) and the presence of the AA-NAT gene (G619A polymorphism) and the CKIϵ gene (S408N polymorphism) influence the prevalence of DSPS amongst Japanese and Brazilian populations.

Effect of the Biofield Energy Treated Proprietary Test Formulation for Sleep Biomarkers in the Unpredictable Chronic Stress (UCS) Animal Model

Sleep biomarkers in brain such as melatonin, BDNF (Brain-derived neurotrophic factor), PGD2 (Prostaglandin D2), leptin, orexin-A, and acetylcholine were evaluated in the unpredictable chronic stress (UCS) rodent model in the presence of Consciousness Energy Healing Treated (the Trivedi Effect®) novel test formulation in male Sprague Dawley (SD) rats using ELISA assay. The test formulation was consisted of minerals (Zn, Fe, Cu, Se, Ca, Mg), vitamins (C, E, B6, B12, D3), β-carotene, ginseng, and cannabidiol isolate. The test formulation constituents were divided into two parts, one part of each ingredient was distinct as the untreated test formulation, while the other portion of the test formulation and a group of animals received Biofield Energy Healing Treatment by a renowned Biofield Energy Healer, Mr. Mahendra Kumar Trivedi. The level of melatonin in groups viz. G5 (Biofield Energy Treated Test formulation) and G7 (15-days pre-treatment of Biofield Energy Treated Test formulation) was significantly increased by 17.6% (p≤0.01) and 16%, respectively as compared with the disease control group (G2). Brain-derived neurotrophic factor (BDNF) level in brain was increased by 5.2% in G7 group as compared with the G4. Prostaglandins D2 (PGD2) level was significantly (p≤0.001) increased by 12.7%, 18.1%, 23.7%, and 30.7% in the G6, G7, G8 (15 days pre-treatment of Biofield Energy Treated Test formulation to the Biofield Energy Treatment per se rats), and G9 (untreated test formulation to the Biofield Energy Treatment per se to the rats) groups, respectively as compared with the G2. The level of leptin after Biofield Energy Treatment and with the test formulation was altered. However, orexin-A level was significantly decreased by 37.1% (p≤0.05), 32.6%, 40.5% (p≤0.05), 44.4% (p≤0.05), and 28.2% in the G5, G6, G7, G8, and G9 groups respectively, as compared with the G2. Similarly, acetylcholine (Ach) level was significantly (p≤0.001) decreased by 42.5%, 49.2%, 40.1%, 47.9%, and 45% in the G5, G6, G7, G8, and G9 groups, respectively as compared with the G2. Overall, the results showed the significant slowdown the stress-related disease progression and its complications/symptoms in the preventive in the Biofield Energy Treatment group per se and/or Biofield Energy Treated Test formulation groups (viz. G6, G7, G8, and G9) comparatively with the disease group. The Trivedi Effect® showed increased level of melatonin and decreased levels of insomnia related brain biomarkers which might be helpful to induce better sleep in human.

The Effect of Weekends and Clock Changes on the Sleep Patterns of Children with Autism: A Study of Historical Records

Background Children with autism spectrum disorders (ASD) often have difficulties settling to sleep and maintaining asleep through the night. Sleep difficulties are linked to challenging behaviour so understanding the causes of these difficulties is vital. Possible explanations are: (1) that irregular innate cycles lead to difficulties maintaining/initiating sleep at the appropriate times; (2) that children with ASD fail to learn from the contingencies that teach neurotypical children to initiate and maintain sleep. If the cycles are innate then small externally imposed changes in routine will not affect the sleep cycle. Methods The sleep records of 46 children with autism and moderate to profound intellectual impairments attending a residential school were examined to identify the effects of spring time change and weekend leave on 1) the times children went to sleep, 2) the length of their sleep and 3) the number of sleep disruptions. Manual staff recordings of the children’s sleep were conducted and data for these variables were analysed using repeated measures analysis of variance. Results A later sleep time was found in children regarding their sleep onset on Sunday after the time change (average onset was 9:57 p.m. ((s.e. = 8.49 minutes) versus 10:17 p.m. (s.e. = 8.19 minutes), with analysis of variance of sleep onset time showing a significant effect (F (3,41) = 5.02, p = 0.005). However, only two out of three comparison groups showed statistically significant effects (March 23rd versus March 30th mean difference = 0.39, p = 0.003; March 30th April 13th mean difference = 0.36, p = 0.03). No statistical difference was found between March 30th versus April 6th or other sleep parameters in any groups (i.e., sleep duration or night time awakenings). Similarly, no change in any sleep parameters (i.e., sleep onset or awakenings) were found when Sundays sleep parameters were compared to Mondays and/or Tuesdays. Conclusions In this small pilot study, small changes of day/night cycles appear to have few effects on the sleep patterns of children with ASD attending a residential school. While no significant sleep pattern change was found in this population due to change of clock times or weekend visits, larger epidemiological studies addressing other unexamined variables to better delineate changes in ASD are needed.

The Effects of Physical Activity on Sleep among Adolescents and Adults: A Narrative Review

Objective: Examining the correlation between physical activity measures and sleep in normal adolescents and adult population. Methods: A systematic review was conducted on the effects of exercise, its intensity, its frequency and its timing and sleep outcome. Using the databases including MEDLINE, PsycINFO and SPORTDiscus, keywords used were “sleep”, “circadian rhythm”, “exercise”, and “physical activity”. Results: To improve the general quality of sleep amongst adolescents and adults, individuals should engage in physical activity at any time of day. It also appears very advantageous to engage in long-term physical activity to maintain the positive effects on sleep. Nonetheless, it seems that sedentary individuals can also benefit from occasional physical activity to improve the quality of their subsequent sleep period. Conclusion: A virtuous relationship between physical activity and sleep may exist in normal adolescents and adults. People should seek to benefit from this link to improve both the quality of their sleep and of their daily functioning. However, larger scale studies, controlling for variables might help to better delineated this relationship.

The Role and Effect of the Comprehensive Music Therapy in Project Sleep: A Multi-Disciplinary Quality Improvement Project.

Sleep is vital for the maintenance of physical health and mental wellbeing. Sleep also plays a cardinal role in the process of healing. It is estimated that 50 to 70 million Americans suffer chronically from sleep disturbances and insufficiency, which not only hinders daily functioning but also adversely affects health, quality of life and longevity.1 Deficient sleep is associated with an increased risk of developing chronic diseases such as hypertension, diabetes, obesity, heart disease, stroke, depression, frequent mental distress, as well as increased mortality, and reduced quality of life and productivity.1,2Aging, medical conditions, pain, and mental illness further aggravate sleep disturbances such as insomnia, sleep fragmentation and daytime sleepiness.3, 4, 5, 6, 7, 8 The a common treatment for sleep disturbances and insomnia is pharmacological therapy. Benzodiazepines may have negative long-term side effects including residual daytime sedation and tolerance development. Withdrawal difficulties may increase dependency. In  the older patient  there are  safety issues related to daytime sedation including increase risk of falls.  With the importance of sleep in hospital recovery and the side effects of sleep medication becoming more widely recognized, there has been an impetus to s use nonpharmacological alternatives , such as music.. This pilot study builds on the potential impact of music’s effectivity by introducing music therapy as a safe, cost effective and culturally sensitive intervention.

Assessment of Cardiac Function and Prevalence of Sleep Disordered Breathing using Ambulatory Monitoring with Acoustic Cardiography – Initial Results from SWICOS

The aim of this study was to assess the use of ambulatory acoustic cardiography during the initial data collection of the longitudinal study of a rural population in Switzerland (n=297, mean age 48.9 ±16.5 years, 57% female). Ambulatory acoustic cardiography non-invasively can assess sleep disordered breathing (SDB) and provides markers of left ventricular systolic and diastolic dysfunction. The percentage of the third heart sound detected during sleep decreased significantly across age groups (age < 40 years, 40-60 years, > 60 years) for both genders (males, p=0.04; females, p=0.02). The percentage of a fourth heart sound detected exhibited an increasing trend for both genders with age suggesting increased diastolic dysfunction with aging. Mean electromechanical activation time (EMAT) during sleep was within the normal range across age groups and both genders (male 93.7 ± 11.6 ms, female 94.6 ± 13.0 ms), and did not vary significantly with age. A large proportion of subjects had a high likelihood of sleep disordered breathing (17.6%). Baseline characteristics categorized by SDB severity indicate increasing age, male gender and being overweight (BMI ≥ 25) to be associated with greater SDB severity. Acoustic cardiography findings categorized by SDB severity reveal increased nocturnal non-dipping heart rate, presence of atrial fibrillation, prolonged QRS duration and QTc interval, increased percentage of fourth heart sound detected, and longer EMAT to be significantly associated with greater SDB severity. Overall, acoustic cardiography detected a very low prevalence of systolic dysfunction, age-related increases in diastolic dysfunction and a moderate prevalence of sleep disordered breathing.

Obstructive Sleep Apnea in Individuals with Down Syndrome: A Meta-Analytic Literature Review

Objective: Individuals diagnosed with Down syndrome (DS) are predisposed to obstructive sleep apnea (OSA). The aim of this study is to assess the prevalence of OSA and factors associated with OSA in this population. Study Design: Systematic literature review and meta-analysis. Methods: Studies of DS and OSA in the English language through May 2017 were reviewed. Since parental report of symptoms has limited validity related to the diagnosis of OSA, only studies using in laboratory polysomnography to diagnose OSA were included. Results: Twenty three studies examining OSA among 1,469 people with DS were found. Among ten studies using community referred samples, 71.5% of people with DS had OSA, compared to 69.6% in referred community samples suspected of having respiratory events. There was an inverse relationship between apnea hypopnea index (AHI) cutoffs and OSA prevalence as higher cutoffs were associated with somewhat lower prevalence. Examining age groups, adults had a higher prevalence of OSA (90.0%) compared to infants (66.5%) and children between 2-21 years of age (69.9%). Oxygen desaturation and gender did not affect prevalence. Although surgery had less effect on successfully treating OSA among DS individuals compared to those without DS in prior studies, lingual tonsillectomy had the greatest effect (mean AHI decrease of 9.0). Conclusion: OSA appears to occur frequently in children and adults with DS. Untreated, OSA may contribute to health problems and premature death, highlighting the potential importance of identifying OSA among people with DS. Even after traditional surgeries (i.e., adenotonsillectomy, tonsillectomy, adenoidectomy), repeating PSG is highly recommended as residual OSA can persist

Intervention Study to Improve Meal Habit, Sleep Habit, Circadian Typology And School Marks in Japanese Elementary School Students

Purpose: An intervention program was performed to improve meal habit, sleep habit and academic marks at school. This program was called “Super Meal Educational Program” which was supported by Japanese Ministry of Arts, Sports, Sciences and Education. Effects of the intervention were evaluated by comparing an integrated questionnaire study held both before and after the intervention of a half year. Method: The intervention program consisted of a lecture on sleep health and diurnal rhythms on children by an university professor, a series of lectures and practical classes by elementary school teachers on foods which can be collected nearby, and also practical courses as excursions to learn traditional meals and several foods resources which are products nearby. Integrated questionnaire study and also government official academic achievement test were performed before and after the intervention for 6 months in May to November, 2015. An integrated questionnaire was administrated to elementary school students of grades 1 to 6 (7-12 years old). For the younger students aged 6-9 years old, parents (mostly mothers) answered the questionnaire instead of their children. The participants consisted of 40 to 55 students in each grade from the first to the six one of the elementary school. The rate of answer to the questionnaire was more than 95% and the number of data used for statistic analysis was 295 before and 286 after the intervention. The integrated questionnaire included questions on the diurnal type scale (DTS), sleep habits, meal habits, and other questions on their environments and life-habits. Result: In the histogram on distribution of the diurnal type scale scores (DTS) before intervention, the students which occupied 25% evening-typed significantly shifted to more morning-typed ones after the intervention (p<0.001). There was significantly negative correlation between DTS scores before intervention and the amount of increased change in DTS during the intervention period (r=-390, r<0.001). For all students in the elementary school, there were no significant changes in sleep quality, bedtime in weekdays, wake up time and sleep hours before and after the intervention period. There was negative correlation between sleep hours in weekday before intervention and increased amount in sleep hours before and after that (r=-0.545, p<0.001). The longer the sleep hours were shifted longer during intervention, the better the academic achievement mark of mathematics was after the intervention (students in the second, third and fifth grades: p=0.001). Conclusion: At least for the students who had been originally evening-typed, the intervention on meal habits may be effective for longer sleep hours and improved school achievement and marking for especially mathematics in elementary schools.

Short-Term and Chronic Effects of Aerobic and Resistance Exercise on Sleep in Young Men

Despite the generally accepted benefits of exercise on sleep there remains limited research on potential differential effects by exercise type. The purpose of the present study was to examine short-term and chronic effects of aerobic and resistance exercise on various sleep quality parameters, as well as sleep duration. Generally healthy, previously sedentary young (27±3 years) men completed a 16-week aerobic and 16-week resistance exercise program in random order separated by a minimum of 6 weeks with no formal exercise. Each exercise program consisted of three supervised exercise sessions per week. Quality and duration of sleep was determined with a multi-sensor device that was worn prior to, during week 1 and week 16 of each exercise program. A total of 8 participants provided valid data on time spent awake after sleep onset, sleep latency, total sleep time, sleep efficiency and time spent in bed for both exercise programs. During week 1, aerobic exercise was associated with a significant decline in sleep latency (-6.5±6.8 min) and time in bed (-39.2±42.2 min) while resistance exercise was associated with a decline in time spent awake after sleep onset (-21.6±16.7 min) and increased sleep efficiency (4.3±4.8 %). Effects were no longer significant after 16 weeks of exercise. These results indicate that aerobic and resistance exercise have beneficial effects on quality of sleep, particularly in the short-term, but the specific exercise-induced changes vary by exercise type.

Obstructive Sleep Apnea Treatment with Epap Nasal Devices: Physiological Principles and Limitations

Expiratory Positive airway pressure (EPAP) devices such as prevent have been used for treatment of obstructive sleep apnea without discussing their limitations. In this short review we discuss the physiological limitations of EPAP devices during inspiration and during expiration. During spontaneous breathing, when EPAP is excessive, the patient would have difficulty breathing in because lung compliance decreases at higher volumes. Furthermore excessive EPAP could lead to progressive trapping of air in the lungs. An ideal EPAP device should allow the patient to adjust the resistance to a comfortable level that would provide EPAP without a progressive buildup in pressure, without compromising tidal volume, without causing CO2 retention, and without disturbing sleep. The use of EPAP devices with adjustable resistance is essential for best results in treatment of obstructive sleep apnea and snoring.

Obstructive Sleep Apneas, Cervical Osteophytosis and Sudden Death: A Paradigmatic Case and a Brief Overview of the Literature

Obstructive sleep apnea (OSA) syndrome is a common disease characterized by partial or complete collapse of the upper airway during sleep secondary to functional or anatomical factors. The gold standard method for OSA diagnosis is an overnight polysomnogram demonstrating repetitive obstructive apneas and hypopneas during sleep. OSA syndrome is associated with cardiovascular diseases, stroke and rarely with sudden death. OSA and cervical spine osteophytes share some common risk factors, and their coexistence may cause mechanic respiratory obstruction with a severe sleep apnea. We present a brief overview on this syndrome, its links to the cervical spine pathology and their combined effect on a patient presenting with neurological signs who suddenly died before an effective treatment was possible to perform. This case highlights how a rapid deterioration of the functional balance may be possible even when a clinical condition has been present, known and unchanged for a long period of time and the need to treat adequately a not-so-innocuous pathology without an excessive delay.

A Comparison Study of the Fitbit Activity Monitor and PSG for Assessing Sleep Patterns and Movement in Children

Background: Despite its expense, labor and intrusiveness, polysomnography (PSG) is the gold standard for diagnosing obstructive sleep apnea (OSA). Recently, commercially available electronic activity monitors, such as Fitbit, have become widely accepted and can provide an estimate of sleep patterns for screening children with possible OSA. A previous study demonstrated Fitbit to be valid compared to PSG in adults. To date, these devices have not been extensively utilized for research in children with sleep disordered breathing (SDB). Objective: To evaluate the validity of the Fitbit activity monitor compared to PSG in children and adolescents with SDB. Methods: Data was collected from 14 children, ages 3 through 11, who were scheduled for a PSG during the study period. Fitbit was worn concurrently during the night of the PSG. Analyses were performed by comparing total sleep time, number of awakenings, sleep efficiency and wake after sleep onset (WASO) Fitbit parameters with the corresponding parameters measured by PSG using Spearman’s rho. Fitbit movement epochs were also compared to PSG epochs showing movement behavior. Results: Pilot data suggest that Fitbit demonstrates a high sensitivity for sleep, a low specificity for wake and a trend suggesting good association of movement measurements. Conclusion: Although Fitbit is not as accurate as PSG for determining wakefulness, it may be a useful screening device to assess gross sleep quality in children. Further studies are indicated to validate these findings.

Subjective and Objective Actigraphic Sleep Monitoring and Psychopathology in a Clinical Sample of Patients with Night Eating Syndrome, with and Without Binge Eating Behaviors

Introduction: Night Eating Syndrome (NES) is a phenomenon combining aspects of both sleeping disorders and Eating Disorders (EDs). To the best of our knowledge, few studies to date have examined NES among patients with EDs. None examined ED patients with and without NES in clinical settings by comparing their sleep disturbances using the new NES diagnostic criteria 1. This study aimed to compare subjective and objective sleep patterns and ED-related psychopathology among NES patients with and without binge eating (BE) behavior. Method: The sample included 71 women, ages 19-62, referred for assessment to a hospital Eating Disorders Clinic. Measures included self-report questionnaires, psychiatric evaluation and actigraph recording for one week. Participants were divided into three subgroups: those with NES without BE behavior (NES-only, n = 17), with both NES and BE behavior (NES-BE, n = 22), and with BE behavior without NES (BE-only, n = 32). Results: Regarding objective sleep monitoring, only one group difference emerged: significantly longer sleep duration for the NES-only group than the other groups. Subjectively, both NES-only and NES-BE groups described significantly more sleep disturbances than the BE-only group. Moreover, only one inter-group difference emerged in ED-related psychopathology: According to new NES diagnostic criteria, the NES-only and NES-BE groups reported significantly higher levels of NES symptoms than the BE-only group. Conclusions: Groups' similarities in sleeping disturbances and psychopathology level and differences between objective and subjective sleep disturbances raise the question of whether NES should be considered part of the BE-only subgroup, calling for further research.

Loneliness and Bedtime Procrastination: Exploring a Model of Interconnectedness Among Young Adults in Germany

Background/Aim This study aimed to examine the relationship between loneliness and bedtime procrastination in young adults in Germany, offering critical insights into the development and maintenance of both conditions. Specifically, it sought to test a new model that connects these two psychological constructs. Materials and Methods A quantitative online survey was conducted with 500 young adults (Mage = 23.3, 83.6% female). Loneliness was assessed using the UCLA Loneliness Scale, while bedtime procrastination was measured using the Bedtime Procrastination Scale. Results The results show a significant positive correlation between loneliness and bedtime procrastination in young adults. Sleep quality was also found to significantly correlate positively with loneliness as well as with bedtime procrastination. Conclusion The findings provide a crucial framework for understanding the relationship between the two psychological constructs of loneliness and bedtime procrastination. Implications for possible future interventions are discussed.

Statistical Study on the Impact of Computer-use on Child-health in the Arab-community

United States of America (USA), Australia, and Canada recommending children computer-use should be imperfect. It obligated a strong influence on children’s lives. The problem was for stuck in the computer-use for a long time per day. The goal was to follow child-health in the Arab-community to notice its influence on the child-health. The method was through a arithmetical study by sending Internet questionnaire and receiving the parents' advice. The results were in the 500 parent answers and 14 comments. The first was "You have a child up to 13 years old?"; 83.6%. The second was "Your child used a computer?"; 80.8%. The third was "Your child used the computer for a long time?"; 67.9%. The fourth was "The computer caused a health problem for your child?"; 70.3%. The fifth was "The computer caused your child mental disorder?"; 68.5%. The sixth was "The computer caused your child trouble seeing?"; 77.2%. The seventh was "The computer caused your child trouble sleeping?"; 72.0%. The eighth 8 was "The computer caused your child feeding problem?"; 69.5%. The ninth was "The computer caused your child family relationship problem?", 72.0%. The tenth was "Prevent your child from using the computer to protect his health?"; 79.0%. It was concluded that the extent of the harms of child computer-use from a health and psychological point. They have young and sensitive tissues. The computer-use affected them, child-health. It was recommended the parents instructions will protect children from the computer-use for long periods. That will reduce child-health and psychological problems.

Healthy Aging and Muscle Dysfunction: Will Melatonin Help?

Background Aging is said to represent a declining state that is possibly especially compromised by sleep issues, and declining melatonin levels. Aim This review examined the idea that aging can be favorably influenced in numerous ways by the addition or maintenance of adequate melatonin levels. Specifically, the impact of melatonin on skeletal muscle was the key topic of interest. Method Reviewed were peer reviewed research and review articles specifically pertaining to healthy aging, melatonin, and muscle associated observations. Results Declining melatonin levels greatly impact multiple essential body systems and tissues. Supplements or interventions that heighten melatonin presence appear to have beneficial impacts on aging in general, and muscle function and structure, in particular. Conclusion The use of melatonin early on in the aging process is likely to produce more favorable long-term outcomes than not in cases of deficiency, and should be further investigated.

Confirming Tonality at Residences Influenced by Wind Turbines

For 5 years, since the start-up of an array of 140 wind turbines, residents have filed complaints with the Ontario Ministry of the Environment (the regulator), and K2 Wind (the operator). Residents complained that the turbines produce a tonal sound, and that the irritation this produced impacted their sleep, their health, and the enjoyment of their property. To confirm tonality from the wind turbines, this research examined over 200 data examples from two families. These families collected data by two independent methods, a continuously recording system, and by making selected audio recordings. The recorded data was correlated with the wind turbine operational performance, and local weather conditions. The correlated data was analyzed for tonality using international standard evaluation methods. The analysis confirmed over 84% correlation between complaints of irritating conditions, and tonality from 5 dB to over 20 dB. Finally, the results showed that the assumption of the regulator to only require assessment of compliance when the resident was downwind of the nearest wind turbine was incorrect. Most complaints arose from other wind directions. Neither was the regulator’s assumption correct that curtailing the wind turbine operation to continue operating at only partially reduced outputs would give remediation. The research concludes that tonality arises consistent with the wind turbine operation, identifying a critical need to revise the practices to prevent chronic irritation. In the original issuance of this paper, the author inadvertently erred by stating that there was a difference between the method for assessing wind turbine tonality of an expert group panel and the method now prescribed by regulations. That statement was incorrect, and the author apologizes for the error and for any confusion it may have caused. The error, miscalculations resulting from the error, where necessary conclusions drawn from erroneous calculations are corrected.

Concordance of Stress Among Married Couples in Urban Area of Nagpur, Maharashtra, India

Background Concordance or husband wife agreement on various factors like sleep, substance use, dietary supplement use, and diseases like obesity and hypertension has been extensively studied worldwide. But the data regarding concordance of stress among married couples in India is lacking. Objective The prime objective of this study was to find out proportion of concordance of stress among married couples in Nagpur, Central India. Methods This was a community based cross sectional study conducted in an urban area of Nagpur. 260 couples in whom both husband and wife were of the age 30 years or more were included. Stress among couples was assessed by Presumptive stressful life events scale (PSLES). Results 82.3% concordance of stress was observed. Getting married, birth of daughter, death of close family member, illness of family member, gain of new family member and going on pleasure trip or pilgrimage were most common events reported by both spouses. Pregnancy of wife was the most common event reported by husbands. Four husbands reported death of spouse as a life event. None of the spouses reported extra marital relation of spouse and conflict over dowry as life time events. Conclusions Concordance of stress was very high among couples. This suggests role of shared marital environmental factors, which could affect stress concordance among couples.

Ehlers Danlos syndrome (EDS) is a collagenic disease that has often been associated with different types of sleep disorders ranging from insomnia to obstructive sleep apnea (OSA). EDS usually has associated fatigue and excessive daytime sleepiness (ES), thus narcolepsy should be excluded as a cause. Literature review suggests a high prevalence of hypersomnia disorders in this population. We present two sporadic cases presenting with typical symptoms of narcolepsy.

Dream theory from the perspective of Islam

Background and Objective: Sleep as a sign of the God wisdom and power, is the agent of stability and tranquility. Sleep has a spiritual dimension. Sincere dreams are the continuation of prophecy and guidance of the God. This study aimed to explain the dream theory from the perspective of Islam. Materials and Methods: This evolutionary study has focused on these questions: “what is the process of dreaming in Islam? Can people control their dreams”? Islamic evidences were taken from the verses and narrations related to the words “soul, sleep, dream and death”, using al-Mu’ayyim, authentic Shi’a commentary books, Bihar al-Anwar, Kofi principles, the Book of Resurrection and life after death. The religious and scientific evidences were analyzed based on the Walker and Event content analysis method. Dream theory was extracted from religious evidence. Results: Based on "revelation epistemology" during sleep, the ‘template of a higher universe’ travels to the unseen world and divine guidance can be provided. People with spiritual health) (owners of the Sound heart) can control the type of their dream, and solve problems by the guidance of God. In the perspective of Islam, there are three types of dreams: sometimes is divine guidance and glory from God, sometimes is sorrow from the devil, and sometimes are conflicts of daily living or past events. Conclusion: Regarding the spiritual aspect of sleep and its deep impact on physical and mental health, it is essential for medical staff to become familiar with dream theory from the perspective of Islam. They should explain to patients and family members the importance and impact of sleep on the acceleration of tissue repair and the treatment of anxiety and depression. To those who are willing to enjoy the divine guidance through the dream, they can teach the Islamic method of dream control.

Newborns’ Cranial Vault: Clinical Anatomy and Authors’ Perspective

Cranial vault is the case surrounding the brain. Its structure differs in newborns than in adults in some aspects. Such differences should be clear for both health and family members. Moreover, the anatomy and embryology of fetal skull take a little attention in the previous literature and textbooks. Therefore, this short review aimed to clarify some aspects of anatomy and clinical importance of cranial vault features in newborns. The newborn vault is formed of multiple separate flat bones connected by fibrous tissues with wide soft gaps called fontanelles. Development of bones of skull vault is closely correlated with the expanding growth of the underlying brain. Such brain shouldn't be struggled by continuous tightening of the newborns' vault. Also, the newborn skull could be affected even by a fixed sleeping position.

The Duration of the Alcohol Hangover

Introduction and aims Previous research has focused primarily on the severity of the alcohol hangover. Much less is known about the duration of the alcohol hangover. The objective of this analysis was to examine the average duration of the alcohol hangover. Design and Methods N=811 Dutch students, aged 18-30 years old, completed a survey on alcohol consumption and their latest alcohol hangover. Overall hangover severity was reported for every two hours after waking up (starting 4am until midnight). Hangover duration was calculated from the moment of stopping alcohol consumption until the first time hangover severity scores reached zero. Results During the drinking session that resulted in their latest hangover they consumed a mean (SD) of 5.9 (6.2) alcoholic drinks, followed by 6.5 (2.0) hours of sleep. The start of the alcohol hangover was not assessed, as it can be assumed that BAC approached zero while participants were sleeping. From the moment of stopping alcohol consumption, the average duration of the alcohol hangover was 18.4 (3.8) hours. For the majority of drinkers, hangover duration ranged from 14 to 23 hours. Alternatively, the hangover lasts approximately 12 hours from the time of waking up. Hangover duration correlated significantly with hangover severity and total sleep time, but no significant association was found with the number of alcoholic drinks that were consumed. Discussion and Conclusions The duration of the alcohol hangover ranged from 14 to 23 hours, with an average of 18.4 hours after stopping drinking, or alternatively, about 12 hours after waking up.

Association of Non Epileptic Seizures and Epilepsy:2 Cases Reports of Masturbation Seizures

Introduction: Non epileptic seizures (NES) are little reported in our country. Case report: We report 2 observations of 4 years old girls, with normal psychomotor development, and frequent paroxystic seizures, characterized by stereotyped movements of the members, occurring during sleep onset or deep sleep. Electroencephalogram Sleep records showed brief discharges of spikes and spikes-waves. Magnetic Resonance Imaging was normal. Suspicion of NES associated with epilepsy was based on the following arguments: mild regression of the seizures frequency after using antiepileptic drug video records of the seizures strongly suggestive of masturbation seizures, masturbation seizures are triggered by the girls and stopped on order, best regression of the seizures frequency after adjunction of trihexiphenidyl. Moreover, the authors make a review of literature on the NES. Conclusion: The early diagnosis of the NES allows to avoid the inadequate treatments and an improvement of the prognosis.

Obesity in Schizophrenia

Over the last three decades, an epidemic of obesity has markedly affected patients suffering from mental illnesses such as schizophrenia. Antipsychotic medications used to treat schizophrenia are considered as major culprits. The aim of this review is to first consider risk factors, to then outline negative sequelae of obesity for this population, and finally to address timing and content of recommended clinical interventions. Medical databases were searched with the terms “”weight,” “obesity,” and “schizophrenia.” Selection of articles was guided by date of publication; recent papers are preferentially cited. The main findings were that, in addition to antipsychotic medications, socio-economics, lifestyle, immune factors, and circadian rhythms also contribute to obesity risk. A barrier to effective health promotion within psychiatry has been the concern that fears about gaining weight might stop individuals with schizophrenia from taking needed antipsychotic medication. Recommendations, therefore, are to keep the dose of antipsychotic medication as low as possible, avoid polypharmacy, encourage healthy eating and physical activity, address sleep problems and substance use, monitor weight, blood pressure, and metabolic parameters regularly, utilize motivational interviewing techniques and peer support, pay special attention to special needs such as those of women during pregnancy, and include bariatric surgery as a potential intervention. Conclusion: Besides careful attention to medication regimens, the literature supports the active encouragement and support of patient self-management strategies to both prevent and manage obesity in schizophrenia.
