Overview

Drug development is the structured progression of a candidate molecule from a validated lead through preclinical characterization, clinical evaluation, and manufacturing toward an approved therapeutic. It encompasses pharmacology, formulation, dose selection, assessment of efficacy and safety, characterization of drug-drug interactions and adverse effects, and the regulatory and quality requirements that govern each stage. Early development relies on integrated in silico, in vitro, and in vivo investigation to define mechanism, exposure, and liability, while preclinical disease models, including in vitro systems for cardiotoxicity and safety testing and stem cell-derived cardiomyocyte assays, help predict clinical performance before human exposure. Toxicology and drug-safety evaluation are continuous priorities that determine whether a candidate advances. Structure-activity relationship analysis, illustrated by the historical development of agents such as the sulfonamides, underpins rational optimization, and proteomic and genomic techniques support target validation, biomarker discovery, and stratification for diagnostics and personalized therapy. Development is increasingly informed by individualized approaches that connect pharmacogenomics to tailored medication and nutrition, by antimicrobial stewardship considerations, and by accelerated programmes such as vaccine development under pandemic conditions. The journal publishes peer-reviewed research spanning these molecular, preclinical, translational, and safety dimensions of bringing new medicines from concept to clinical use.
